HCBS Quality Measure Set

States must report on the outcomes of their services using these measures. States will be required to ask certain survey questions annually and report on members' answers. Cadence has this survey built into our system and easily prompts users to complete the survey annually. Results are collected from members and measures are automatically populated. Our reporting structure allows for easy presentation of populated measures to CMS.

Functional needs Assessment

This annual assessment must be provided to all members as a part of their person-centered plan. Cadence has this assessment built into our system and can easily prompt case managers to complete as a part of the person-centered planning process. Results are collected and the person-centered plan is automatically populated. Our reporting structure allows for easy dissemination to members and their families.
